INSTRUCTIONS

All Stateline Community Foundation applications must be completed online. You will receive an email acknowledgement once the application has been submitted. All additional materials must be submitted in one packet to the Stateline Community Foundation, ATTN: Christine Green, 121 West Grand Avenue, Beloit, WI 53511 no later than 5:00 p.m. on March 15th, 2008. Incomplete or late applications will not be considered.

Please carefully follow these instructions:

• Review the eligibility criteria for each scholarship. Do not apply for scholarships for which you do not qualify.
• Complete the application form and select the scholarship(s) for which you want to be considered.
• Review the application, make any necessary revisions and submit the application.
• The following attachments must then be submitted to the SCF in one packet:
  • Completed checklist
  • Resume
  • Official transcript in a sealed envelope
  • Two letters of recommendation (non-family)
  • Student Aid Report (SAR) obtained form FAFSA (www.fafsa.ed.gov )
  • Senior picture
  • Any additional documentation required of a specific scholarship
  • Do not staple your application materials. Please use paperclips only
